Minnesota requires providers to publish these notices. Available in other languages and formats on request.
What you're entitled to
- To be treated with courtesy and respect.
- To be free from abuse, neglect, exploitation, and retaliation.
- To help write your own treatment plan.
- To refuse services, and to be told what that means.
- To know the qualifications of the staff who serve you.
- To request a different staff member.
- To privacy and confidentiality of your records.
- To see, copy, and request corrections to your record.
- To raise a grievance and get a written response.
- To be told in writing before services end.
- To have a health care directive honored.
- To contact the Ombudsman at any time.
How to raise a concern
Raising a concern will never affect your services.
- Tell any staff member, or send us a message.
- We confirm receipt and tell you who is reviewing it.
- We investigate and respond in writing.
- If you disagree with the outcome, you may contact the agencies listed here.

Your health information
This is a summary. Our Notice of Privacy Practices sets it out in full.
- We use your information to provide, coordinate, and bill for care.
- We share it only with your written consent, except where the law requires it.
- You may see and copy your record and ask us to correct it.
- You may revoke a release of information at any time, in writing.
When we must report
Our staff are mandated reporters. We must report suspected maltreatment of a vulnerable adult or a child, and we act when someone is at immediate risk of serious harm.
